He was born 26 March 1939 in Santa Rita, Grant, New Mexico. On 1 July 1956 he married Carol Sue Sullivan in Ashland, Jackson, Oregon. Together they had five children:

Jerry died 17 Feb 1967 in Asland, Jackson, Oregon. Cause of death was suicide. His death records can be found in the Oregon Death Index and in the Social Security Death Index. He is buried in Ashland.
